What blessings are recited on bread, cookies, hot chocolate, tea, and banana?

Question

What are the precise blessings for bread, cookies, hot chocolate, tea, and banana? Also, when I eat a sandwich, I do not recite the 'Hamotzi' blessing but do say the Birkat Hamazon; does that count? Or is it the other way around? Thank you in advance.

On bread, one recites 'Hamotzi' followed by Birkat Hamazon. For cookies, one recites 'Borei Minei Mezonot' and afterward the blessing for sustenance, Birkat Hamazon. For hot chocolate and tea, one recites the blessing 'Shehakol' and afterward the blessing 'Borei Nefashot.' For a banana, the blessing is 'Borei Pri Ha'adama,' followed by 'Borei Nefashot.'

Even if one forgets to recite Birkat Hamazon, the importance of the first blessing (on the food) is not diminished; similarly, each mitzvah is measured on its own merit.
